1. Applicant shall apply in Form P.Go.CA.

2. Fees Rs.2000/-

3. Proof for Residence.

4. Proof for Place of parking.

5. Declaration about the place of parking or parking place agreement with the premises owner.

6. Facility to undertake minor repair either by own or contract with a reputed firm (workshop agreement).

7. Declaration about godown facility or godown agreement with any premises owner.

8. In case of Companies/Firms, attested copy of Registration Certificate of the Company.
